Track injuries and concussions for Canada Snowboard members. A coach or official files an accident report, the member's membership goes back to Pending Requirements, and the member's medical clearance completes the requirement again once an admin reviews it.
- An accident report puts the membership on hold. The athlete’s injury and concussion requirement becomes incomplete, the membership goes back to Pending Requirements, and the member is told by email that it’s no longer issued.
- A medical clearance lifts the hold. The member submits proof they can snowboard again. Once an admin reviews it and updates the member’s record, the requirement is complete again.
- A pending requirement doesn’t stop event registration. Event and competition organizers must check each registrant’s membership status on the day of the event.
- Canada Snowboard sets up both forms for you. Each provincial or territorial sport organization (PTSO) has its own Event Dashboard for each form, one per season. The only thing you usually change is who receives the notification emails.
Manage injury and concussion tracking for Canada Snowboard: a coach or official reports an athlete’s accident, a PTSO admin reviews it, and the member submits a medical clearance to return to sport.
-
A coach or official reports the accident
They open the Athlete Accident Report Form from their PTSO, select Register Someone Else and look up the athlete. When the athlete’s details match, they choose to use the existing profile. They answer the form questions, continue to the cart and select Submit & Complete.
-
Review the accident report
The admin responsible gets an email for each report. Open the registration details from the email to read it. The athlete’s Injury/Concussion Tracking (Membership Requirement) attribute has already switched to "NO, the member CURRENTLY has an injury/Concussion". Canada Snowboard and the PTSO admin can review it and change it if needed.
-
The member sees the missing requirement
When the member signs in and opens their membership from My Pages → Memberships, the injury and concussion requirement shows as incomplete on the Requirements tab. How to Resolve explains what to do, with an option to submit their medical clearance.
-
The member submits a medical clearance
The option to submit a medical clearance opens the Return to Sport: Medical Clearance Submissions form. There, the member uploads proof that they can practise snowboard again.
-
Review the medical clearance
The admin responsible gets an email for each submission. Open the registration details from the email and review the proof. If it’s acceptable, update the member’s Injury/Concussion Tracking (Membership Requirement) attribute to "YES, the member is CURRENTLY Injury/Concussion free". The requirement switches to complete.
Set up the forms each season
Each PTSO has two Event Dashboards per season, one for each form, named with the season first:
- Athlete Accident Report Form, for coaches and officials.
- Injury/Concussion Tracking - Return to Sport: Medical Clearance Submissions, for members.
To get them ready for a new season:
-
Find the dashboard
On the Admin Dashboard, select Events → Events and search for the form’s name.
-
Clone it for the new season
Clone last season’s dashboard to create the new season’s form. See Clone an Event Dashboard.
-
Check who’s notified
On the new dashboard, open the notification email settings. Edit the existing notification to change who receives it, or copy its content into an extra notification. See Configure notification emails on Event Dashboard.
-
Share the new links
Send the new accident report form link to your coaches and officials, and make sure members can reach the new medical clearance form.
